Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide compassionate, hands-on care to patients at home, in assisted living, or care facilities. You will collaborate with an RN Case Manager and follow each patient’s plan of care to ensure comfort and dignity.

The role is full-time and field-based, with a Monday–Friday, 8:30 AM–5:00 PM schedule. Competitive pay and benefits accompany opportunities to grow within our supportive team.

Qualifications

• Active CNA certification required.
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice is a plus.
• Strong communication skills, including written, verbal, and active listening.
• Excellent customer service, problem solving, and time management skills.

Responsibilities

• Provide personal care with activities of daily living (bathing, dressing, grooming, toileting, skin care).
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under RN supervision.
• Provide comfort and companionship to patients and support to families.
• Maintain electronic medical records.

Pay: $18 – $23 per hour

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

• Strong communication skills, including written, verbal, and active listening
• Excellent customer service, problem‑solving, and time management skills
• Compassion, patience, and the ability to handle emotional situations with care
• Strong understanding of professional boundaries
• Ability to lift 50 lbs. and good understanding of body mechanics
• Valid driver’s license and current auto insurance (required)
