Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide personal care and support to patients in a hospice setting. The role includes collaboration with a dedicated RN Case Manager, maintaining EMR records, and delivering comfort and dignity to patients during their final days.

Qualifications

CNA license required.
12 months of caregiving experience in Home Care/Assisted Living/Hospice or similar settings.
Current CPR certification preferred.
Excellent customer service skills.
Strong written and verbal communication and active listening.
Problem-solving skills and time management.
Ability to work in an electronic medical record system.
Ability to lift 50 lbs and understand body mechanics.
Strong understanding of professional boundaries.
Valid driver's license required.
Valid auto insurance required.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, etc.).
Review patient care plans and assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition; maintain EMR notes.
Work under the direction of an RN Case Manager.
