Job summary

Moments Hospice is seeking a compassionate CNA to support patient comfort and dignity during their final days. You will work closely with a dedicated RN Case Manager, performing personal care and maintaining electronic records in a supportive team environment.

The role includes a company car with fuel and insurance covered, a cell phone, and a full benefits package with health, dental, vision, PTO and a 401(k) plan. Immediate impact on patient care is emphasized.

Qualifications

CNA license is required.
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
Current CPR certification is preferred.
Excellent customer service and strong communication skills.
Problem-solving, time management and ability to lift 50 lbs.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans and assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition and maintain electronic medical records.
Work under the direction of a Case Manager RN.
