Job summary

Moments Hospice is seeking a compassionate CNA to support patient comfort and dignity in their final days. You will perform personal care, review care plans, and collaborate with a Case Manager RN.

Qualifications

CNA license required.
12 months of caregiving experience in Home Care/Memory Care/Skilled Nursing/Hospice or Hospital required.
Current CPR certification preferred.
Excellent customer service skills.
Strong communication (written, verbal, active listening).
Problem-solving skills.
Time management skills.
Ability to work in an electronic medical record system.
Ability to lift 50 lbs.
Good understanding of body mechanics.
Strong understanding of professional boundaries.
Valid driver's license required.
Valid Auto Insurance required.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
