Qualifications

CNA license required and actively licensed.
12 months caregiving experience in home care, assisted living/memory care, skilled nursing/nursing home, hospital, or hospice.
Current CPR certification preferred.

Ability to manage time and prioritize tasks in a fast-paced environment.

Responsibilities

Perform personal care activities for daily living (bathing, dressing, grooming, etc.).
Review patient care plans and follow directions from the RN Case Manager.
Assist with lifting/transferring patients and mobility.
Assist with range of motion, positioning, and ambulation.
Assist with dining and nutrition; maintain records in EMR.

Qualifications:

Certified Nursing Assistant (CNA) license (required)
12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
Current CPR certification (preferred)
Excellent customer service skills
Strong communication (written, verbal, and active listening)
Problem-solving skills
Time management skills
Ability to work in an electronic medical record system
Ability to lift 50 lbs.
Good understanding of body mechanics
Strong understanding of professional boundaries
Valid driver's license (required)
Valid Auto Insurance (required)
