Job summary

Moments Hospice is seeking a compassionate CNA to support patients in daily living activities, under the guidance of a Case Manager RN. You will provide personal care, assist with mobility, document care in EMR, and collaborate with the RN Case Manager to ensure patient comfort and dignity.

The role includes a competitive salary, company car with fuel/insurance, cell phone, full benefits, and generous PTO. Requires a valid CNA license and patient-care experience; travel required across service

Qualifications

• CNA license is required.
• 12 months of caregiving experience in home care, assisted living/memory care, skilled nursing/nursing home, hospital or hospice.
• Current CPR certification is preferred.
• Excellent customer service skills.
• Strong communication (written, verbal, active listening).
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver’s license required.
• Valid auto insurance required.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans
• Assist with lifting and transferring patients
• Assist with range of motion, positioning, exercise, and ambulation
• Assist with dining and nutrition
• Maintain electronic medical records
• Work under the direction of a Case Manager RN
