Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide high-quality personal care to patients on the road. The role includes assisting with daily living activities, following care plans, and collaborating with an RN Case Manager.

Qualifications

Must hold a valid CNA license and be in good standing.
Minimum 12 months of caregiving in relevant settings.
Current CPR certification preferred.
Excellent customer service skills.
Strong written and verbal communication; active listening.
Strong problem-solving abilities.
Solid time management and organization.
Experience using an electronic medical records system.
Ability to lift 50 lbs.
Understanding of body mechanics and safe patient handling.
Knowledge of professional boundaries.
Valid driver's license and valid auto insurance.

Responsibilities

Provide daily personal care activities (bathing, grooming, dressing, toileting).
Review and follow patient care plans.
Assist with lifting, transfers, and mobility.
Assist with range of motion, positioning, and gait.
Support dining and nutrition needs.
Maintain accurate electronic medical records.
Work under the supervision of a Case Manager RN.
