Job summary

Moments Hospice is seeking a dedicated CNA to provide compassionate in-home care, supporting patients’ comfort and dignity. The role includes personal care tasks, care-plan coordination, and collaboration with an RN Case Manager.

Qualifications

CNA license required.
12 months of caregiving experience in relevant settings.
CPR certification preferred.
Excellent customer service skills.
Strong communication skills (written and verbal).
Good problem-solving and time management abilities.
Ability to use an electronic medical record system.
Ability to lift up to 50 lbs.
Good understanding of body mechanics.
Knowledge of professional boundaries.
Valid driver's license required.
Valid auto insurance required.

Responsibilities

Perform personal care activities (bathing, dressing, grooming, toileting, skin care).
Review patient care plans.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition.
Maintain electronic medical records.
Work under the direction of a Case Manager RN.
