Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to join our on-the-road care team in Saint Cloud, MN. You will provide personal care, review patient plans, and support patients with dignity during their final days under the supervision of an RN Case Manager.

We offer a competitive hourly rate of $18–$23 plus a fleet car benefit, fuel and insurance, a company cell phone, comprehensive health coverage, generous PTO, and a 401(k) with company matching.

Qualifications

CNA license required
12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice
Current CPR certification preferred
Excellent customer service and communication skills
Able to lift 50 lbs and use proper body mechanics

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.)
Review patient care plans
Assist with lifting and transferring patients
Assist with range of motion, positioning, exercise, and ambulation
Assist with dining and nutrition
Maintain electronic medical records
Work under the direction of a Case Manager RN

Qualifications:
