Job summary

Moments Hospice in Green Bay, WI is seeking a compassionate Certified Nursing Assistant to join our on-the-road care team. You will provide essential personal care, review care plans with an RN Case Manager, and support patients with daily living activities and mobility tasks.

Qualifications

Must hold an active CNA license or certification.
Includes at least 12 months of caregiving experience in Home Care, Assisted Living, Skilled Nursing, Hospital, or Hospice.
CPR certification preferred.
Excellent customer service and strong communication skills.
Ability to manage time effectively and solve problems.
Experience with electronic medical records.
Requires valid driver's license and auto insurance.

Responsibilities

Provide personal care for daily living activities (bathing, grooming, dressing, toileting, skin care).
Review and follow patient care plans under RN Case Manager.
Assist with lifting, transferring, and mobility as needed.
Assist with range of motion, positioning, exercise, and ambulation.
Assist with dining and nutrition needs.
Maintain accurate electronic medical records.
Collaborate closely with the RN Case Manager.

Qualifications:
