Job summary

Moments Hospice is seeking a compassionate CNA to provide personal care and support under an RN Case Manager. The role includes lifting, mobility assistance, and documenting in an electronic medical record while delivering dignified patient care during final days.

The position offers a competitive hourly rate ($18-$23) with a company car, gas, insurance, and a cell phone, plus comprehensive benefits, generous PTO, and 401(k) with company matching.

Qualifications

• CNA license required.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing, Hospital or Hospice.
• Current CPR certification preferred.
• Excellent customer service skills.
• Strong communication (written, verbal, and active listening).
• Problem-solving skills and time management.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs and understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license required.
• Valid Auto Insurance required.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
