Moments Hospice is seeking a qualified CNA to provide compassionate in-home care on the road. You will assist with daily living activities, review care plans, and collaborate with a dedicated RN Case Manager to ensure patient comfort and dignity during their final days.

Requirements include a CNA license, 12 months of caregiving experience, and strong communication. A valid driver’s license and Auto Insurance are required; CPR certification is preferred.

Qualifications

• CNA license required.
• 12 months caregiving experience in Home Care, Assisted Living, Skilled Nursing, Hospital or Hospice.
• Current CPR certification preferred.
• Ability to lift up to 50 lbs and maintain good body mechanics.
• Valid driver’s license and Auto Insurance required.

Responsabilités

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice (required)
• Current CPR certification (preferred)
• Excellent customer service skills
• Strong communication (written, verbal, and active listening)
• Problem-solving skills

• Ability to work in an electronic medical record system
• Ability to lift 50 lbs.
• Good understanding of body mechanics
• Strong understanding of professional boundaries
• Valid driver's license (required)
• Valid Auto Insurance (required)
