Job summary

Heritage Woods of Manteno is seeking a dedicated CNA to provide individualized care aligned with each resident’s care plan. You will assist with Activities of Daily Living, promote independence and preserve residents’ dignity.

Qualifications

• High School graduate or GED equivalent required.
• Current CNA certification from an accredited program.
• Excellent communication and interpersonal skills.
• Must demonstrate ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident’s care plan.
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner.
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences.
• Comply with all rules and regulations regarding confidentiality and privacy of resident information.
• Perform any other duties as assigned.
