Job summary

Heritage Woods of Yorkville is seeking a caring CNA to deliver individualized resident care per the care plan, assisting with Activities of Daily Living (ADLs) while promoting independence and dignity.

You will report health status changes to the clinical team, engage residents and families with compassion, participate in ongoing education, and uphold confidentiality; a comprehensive benefits package includes health, dental, vision, 401(k) matching, life insurance, and paid time off.

Qualifications

• High School graduate or GED equivalent required.
• Current certification from an accredited CNA program.
• Exceptional communication and interpersonal skills.

Responsibilities

• Deliver individualized resident care per the care plan.
• Provide assistance with ADLs while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate manner.
