Job summary

Heritage-Woods-of-Bolingbrook in Bolingbrook, IL seeks a dedicated CNA to provide individualized care per residents’ plans and assist with daily living activities, promoting independence and dignity.

You will monitor health status, communicate with residents and families, and participate in ongoing education while upholding privacy and confidentiality standards. This role emphasizes compassionate, respectful care within a collaborative team.

Qualifications

• High School graduate or GED equivalent required.
• Certified Nursing Assistant (CNA) certification required.
• Excellent communication and interpersonal skills.

Responsibilities

• Deliver exceptional and individualized care per each resident’s care plan.
• Assist with Activities of Daily Living (ADLs) while promoting independence and dignity.
• Report changes in resident health status to the clinical team.
• Connect with residents and family members in a compassionate and impactful manner.
