Duration: 13+ weeks with long‑term extension based on performance
Job Description
The Case Worker is responsible for conducting assessments, supporting members with social and health‑related needs, and ensuring seamless coordination of services. This role requires strong communication skills, the ability to work independently, and a commitment to maintaining compliance with all regulatory standards.
Key Responsibilities
- Conduct interviews with members to complete initial assessments and referrals.
- Manage a caseload of social service recipients, ensuring timely follow‑up and ongoing support.
- Perform home visits to assess members’ needs and document observations.
- Schedule and coordinate all home visits independently, maximizing efficiency and productivity.
- Use the Home Visit Assessment Tool during all in‑person or telehealth visits.
- Conduct telehealth home visits when a member declines in‑home visitation.
- Identify and elevate clinical or social concerns to the assigned Care Manager.
- Document all interactions accurately and promptly in the Disease Care Management System (DCMS).
- Provide clients with printed educational materials as needed.
- Assist members with care coordination tasks such as:
- Scheduling appointments
- Arranging transportation
- Addressing medication issues (prescriptions, refills)
- Coordinating DME requests
- Complete other support tasks as assigned by leadership.
- Ensure compliance with HIPAA and all confidentiality guidelines at all times.
Required Qualifications
- Associate's Degree
- Minimum of one (1) year of relevant experience in case management, social services, or a related field.
Preferred Qualifications
- Bachelor’s Degree in Social Work, Human Services, Psychology, or a related discipline.
- Experience conducting home visits or working with high‑needs or vulnerable populations.
