The Case Manager is responsible for coordinating and managing services for clients to ensure they receive appropriate care and support. This role involves assessing client needs, developing care plans, facilitating access to resources, and monitoring progress to promote overall well‑being and positive outcomes.
Base Pay Range
$26.00/hr - $28.00/hr
Key Responsibilities
- Conduct comprehensive assessments of clients’ needs, strengths, and goals.
- Develop, implement, and regularly update individualized care or service plans.
- Coordinate and connect clients with appropriate resources and services, such as healthcare, housing, counseling, financial aid, or rehabilitation programs.
- Monitor and evaluate client progress, making adjustments to plans as needed.
- Serve as a liaison between clients, families, service providers, and external agencies.
- Advocate for clients to ensure they receive adequate services and support.
- Maintain accurate and up‑to‑date case notes and documentation in compliance with organizational and regulatory standards.
- Ensure services provided align with ethical standards, laws, and agency policies.
- Participate in case reviews, team meetings, and training sessions as required.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Nursing, Human Services, or a related field (Master’s preferred in some roles).
- Relevant certification or licensure may be required depending on the field (e.g., RN, LCSW, CCM).
- Experience in case management or a related field is often preferred.
