Job summary

TURN in Oceanside, CA is seeking a Regular Full time over 30 hrs professional to coordinate access to medical, educational, social, vocational, housing, and more. The role focuses on referrals, coordination with health providers, and advocacy for clients’ best interests.

The job requires a High School Diploma, CA driver’s license, and SUD registration, with preference for experience in mental health/substance use services and familiarity with MS Office and EHR systems.

Qualifications

  • High School Diploma or equivalent is required.
  • Active SUD registration is required at minimum.
  • Valid CA driver's license and current auto insurance required.
  • Experience providing mental health and/or substance use disorder services preferred.
  • Knowledge of Microsoft Office and EHR software preferred.
  • Tuberculosis test prior to hire and annually thereafter.
  • CPR/First Aid certification preferred, bi-annual thereafter.

Responsibilities

  • Provide case management with crisis intervention, discharge plan, group & individual work, mental health/substance use disorder counseling, screening, and assist clients in meeting basic needs and symptom management.
  • Complete accurate and timely documentation, meet minimum billable requirements for direct client services if contractually required.
  • Facilitate placement including pre-placement visits, and transportation as needed.
  • Shift coverage including part of the on-call team.
  • Provide field-based services as required by program and client need.
  • Maintain confidentiality of client’s care.
  • Participate in scheduled program meetings.
  • Prepares and maintains client’s case management goals and facilitates monthly housing meetings.
  • Maintains daily log of client contacts. Records data input of client’s demographic information
  • Works closely with program direct service staff to coordinate appropriate housing options for clients relative to functioning level and individual needs and preferences
  • Provides outreach in the community and agencies and attends meetings to provide program education
  • Other relevant duties as assigned.

JOB SUMMARY

Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.
