TURN is seeking a qualified Case Manager to support clients through crisis intervention, multi-disciplinary care coordination, and discharge planning. You will advocate for whole-person care, coordinate referrals, and assist with transportation and housing needs across program lines.
Responsibilities include documentation, on-call rotation, field-based services, and maintaining client confidentiality while collaborating with health and social service providers.

Recommend and assist access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to next level care, coordinate with physical health providers for whole-person care, and advocate in the best interest of clients.
