Case Manager

Job summary

TURN is hiring a Case Manager to coordinate access to medical, educational, social, vocational, housing, and related services to support program goals. You will initiate referrals to next level care, collaborate with health providers for whole-person care, and advocate for clients.

Requirements include a Master's degree (or Bachelor's with four years in social services), CA driver’s license, and experience in mental health or substance use services.

Qualifications

• Master's degree or Bachelor's with four years in social services required.
• SUD CM certification preferred and registration required.
• Housing track requires AA in social services or related field, or HS diploma with 5 years' experience.
• Valid CA driver's license and current auto insurance are required.
• Experience in mental health and/or substance use services preferred.
• Bilingual abilities preferred.
• Knowledge of MS Office and EHR systems is preferred.

Responsibilities

• Provide case management with crisis intervention, discharge planning, group and individual work, counseling, screening, and assisting clients with basic needs and symptom management.
• Complete accurate and timely documentation; meet minimum billable requirements for direct services if contractually required.
• Facilitate placement including pre-placement visits and transportation as needed.
• Shift coverage including part of the on-call team.
• Provide field-based services as required by program and client needs.
• Maintain confidentiality of clients' care.
• Participate in scheduled program meetings.
• Other duties as assigned.

• Cultural competency understanding required.
• Bilingual preferred.
• Knowledge of Microsoft Office and EHR software preferred.
