Job summary

TURN in Oceanside, CA is seeking an Intake & Eligibility Coordinator to assess and connect clients with medical, educational, social, and housing resources, coordinating with health providers to support whole-person care.

This part-time role involves conducting assessments, maintaining timely documentation, coordinating referrals, and supporting a recovery-oriented, confidential service environment.

Qualifications

  • Master’s degree and CA BBS registration or license (MFT, LCSW, LPCC) required.
  • Valid CA driver’s license and current auto insurance per contract requirements.
  • Experience providing mental health and/or substance use disorder services preferred.
  • Knowledge of Microsoft Office and EHR software preferred.

Responsibilities

  • Provide assessments, mental health evaluations, and therapy coordination.
  • Complete accurate and timely documentation; meet minimum billable requirements for direct client services.
  • Facilitate placement including pre-placement visits and transportation.
  • Shift coverage including part of the on-call team.
  • Provide field-based services as required by program and client need.
  • Maintain confidentiality of client’s care.
  • Participate in scheduled program meetings.
  • Other relevant duties as assigned.
  • Conduct initial clinical assessments and eligibility screenings for prospective members.
  • Complete annual assessment updates and maintain accurate clinical records.
  • Collaborate with members to identify strengths, goals, and support needs using a recovery-oriented approach.
  • Coordinate referrals and communicate with internal and external partners as appropriate.
  • Support a welcoming, strengths-based environment that promotes hope, dignity, member choice, and community inclusion.
  • Ensure compliance with program, funding, and documentation requirements.
  • Participate in regular clinical supervision and consultation to support quality care and professional development.

JOB SUMMARY

Recommend and assist with access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to the next level of care, and coordinate with physical health providers to ensure whole-person care, and advocate in the best interest of clients.

RESPONSIBILITIES
