Job summary

The Salvation Army in Macon, GA is seeking a dedicated Case Management professional to interview applicants, conduct needs assessments, and develop intake and case plans for eligible clients. The role includes maintaining case records, documenting progress in HMIS, and coordinating life skills programs to empower clients to cope with their environment.

Qualifications include two years of college coursework in Social Work or related field and at least two years in a social/public service setting.

Qualifications

• Two years of college coursework in Social Work, Behavioral Science, or a related field.
• Two (2) years' experience working in a social or public service environment with experience assisting the public.
• Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Responsibilities

• Interviews applicants to determine eligibility for program enrollment and conduct needs assessment.
• Develops intake and case plans for clients and completes necessary paperwork with goals/budgets.
• Prepares and maintains case records and logs; enters information into HMIS.
• Meets regularly with clients to discuss progress and document outcomes; enforces house guidelines when needed.
• Plans and facilitates life management classes to help clients gain functional skills.
