Job summary

The Salvation Army is seeking a case manager to interview and assess clients for the Pathway of Hope program and establish long-term, individualized goals. You will document case progress and coordinate with external resources to support clients’ stability.

Responsibilities include maintaining HMIS records, assisting with benefits access, and providing direct support to residents while promoting self-sufficiency in a shelter environment.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or related field.
• Minimum three years of direct case management experience with social services.

Responsibilities

• Interview applicants for Pathway of Hope enrollment and assess eligibility.
• Develop long-term, client-specific goals and action plans.
• Maintain and update case records and HMIS data.
• Coordinate referrals to external services and resources.
• Provide transportation support and assist with income and benefit access.
