Job summary

SALVATION ARMY is seeking a dedicated Case Management Specialist in Nashville to provide structured, long-term services to clients in a life management program. You will assess needs, develop individualized plans, and advocate for access to housing, income supports, and community resources.

Qualifications include a Bachelor's degree in Social Work or related field and at least three years of experience delivering direct case management, with strong records, mentoring, and counseling skills.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science, or a related field.
• Three years of progressively responsible experience providing direct case management services.
• Ability to develop comprehensive, long-term action plans for recovery using diverse resources.

Responsibilities

• Locates and arranges temporary housing for eligible clients; coordinates lodging with local shelters, lodges, churches, etc.
• Meets regularly with clients to discuss progress and documents records; handles incident reports and enforces house guidelines.
• Processes rent and utility financial assistance for eligible clients; coordinates with utilities and landlords.
• Provides direct assistance in obtaining income, benefits, and resources to enhance clients' psychosocial well-being.
• Greets and interviews clients to determine eligibility for assistance and assists with applications; records disposition and relevant information.
• HMIS experience preferred.
