Job summary

Public Health Management Corporation seeks a Case Manager I for the Safety & Responsibility Program (SRP). The role supports outpatient activities, schedules intakes for adults and youths, conducts chart audits, and collects performance data.

Reports to the SRP Operations Manager. The ideal candidate has a Bachelor’s degree in a related clinical field and at least two years of experience in public health or social services.

Qualifications

• Bachelor’s degree in a field related to clinical work required.
• At least two years of experience in a related field (public health, public assistance, mental health, social services, case management).
• Strong written and oral communication; ability to meet timelines; analytical and troubleshooting skills.

Responsibilities

• Facilitate tracking of youth participants admitted/maintained in SRP and YSRP.
• Communicate with stakeholders regarding referrals and collect participant information.
• Evaluate and determine payer information for incoming evaluations.
• Schedule intake appointments with participants or guardians.
• Complete intake documentation in Credible and post-evaluation paperwork.
• Coordinate case management activity under supervision.
• Assist supervisor with admin support to clinical staff.
• Disseminate court and monthly reports to Probation and Court Appointed Representatives.
• Assist in developing and implementing new program strategies.
• Generate reports on performance against quality indicators.
