Job summary

Joseph J. Peters Institute (JJPI) seeks a Case Manager for the Safety & Responsibility Program. The role involves supporting outpatient activities, coordinating intake for adults and youth, and completing chart audits. The position reports to the SRP Operations Manager.

Qualifications

• Bachelor’s degree in a clinical field is required.
• Two or more years of related experience (public health, mental health, social services, case management, etc.).
• Experience coordinating intake, data collection, and reporting in a clinical setting preferred.

Responsibilities

• Facilitate tracking of youth admitted/maintained in SRP and YSRP.
• Communicate referral details and collect participant information from stakeholders.
• Evaluate and determine payer information for incoming evaluations.
• Schedule intake appointments with participants or guardians.
• Complete intake documentation in Credible and process post-evaluation documents.
• Coordinate case management activities under supervision.
• Assist clinical staff with administrative tasks.
• Collect and disseminate court and monthly reports to Probation/Court Representatives.
• Develop and implement new program strategies.
• Generate reports on quality indicators and performance.
• Assist in training and QA needs of the agency.
• Support Adult Outpatient program as needed.
• Assist other facility departments as needed.
