Job summary

Joseph J. Peters Institute, a nationally recognized provider serving trauma survivors, seeks a Case Manager for the Safety and Responsibility Program.

The role supports outpatient activities, schedules intake appointments for adults and youths, completes chart audits, and collects and disseminates performance data. The position reports to the SRP Operations Manager and involves coordinating case management tasks with clinical staff.

Qualifications

• Bachelor’s degree in a field related to clinical work is required.
• At least two years of experience in related fields such as public health, mental health, or social services.
• Strong written and verbal communication skills and ability to work under supervision.

Responsibilities

• Facilitate tracking of youth participants admitted/maintained in SRP and YSRP.
• Communicate with stakeholders regarding referral details and collect participant information.
• Evaluate payer information for incoming evaluations.
• Schedule intake appointments for participants and guardians.
• Complete intake documentation in Credible and post-evaluation paperwork.
• Coordinate case management activity under supervision.
• Provide administrative support to clinical staff.
• Collect and disseminate court and monthly reports to Probation/Court Appointed Representatives.
• Assist in developing new program strategies and quality improvement.
• Generate reports on performance against quality indicators.
• Participate in training and education program development for QA needs.
• Assist Adult Outpatient program as needed.
• Support other facility departments as needed.
