Banner Rehabilitation Hospital in Phoenix, AZ is hiring a full-time Case Manager to coordinate inpatient rehab care and discharge planning across the continuum. Licensure per state guidelines or a health or human services degree is required; CCM certification is preferred.

Qualifications

• Assess psychosocial needs and provide discharge interventions.
• Manage clinical interventions and discharge planning processes.
• Oversee payer/referral management and fiscal aspects.
