Job summary

Valley View Medical Center, part of Lifepoint Rehabilitation, is seeking a Case Manager (CM) Rehabilitation to coordinate patient care, perform assessments within 24 to 72 hours, and lead discharge planning in collaboration with the care team.

The role requires a bachelor's degree and current licensure in nursing or social work, with CCM or ARN/ACM certification preferred; inpatient experience is highly valued. We offer comprehensive benefits and opportunities for professional growth.

Qualifications

• Bachelor's degree required.
• Current RN or Social Work licensure or healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
• Certification in Case Management preferred (CCM; ARN; ACM; RN-BC).
• Minimum of two years social work or case management experience in an inpatient setting preferred.

Responsibilities

• Perform intake assessments within 24–72 hours of admission.
• Coordinate discharge planning including home health, equipment, and follow-up care.
• Maintain thorough documentation and communicate with payers and HIM as required.
• Collaborate with nursing, therapy, and other departments to optimize patient care and outcomes.
• Lead or participate in care conferences and interdisciplinary team meetings.
