Qualifications

• RN license in state worked.
• Bachelor’s degree in case management or health care.
• MSW with LMSW/LCSW or progress toward licensure within 6 months.

Responsibilities

• Provide comprehensive care coordination for assigned patients.
• Conduct initial assessments and participate in multi-disciplinary rounds.
• Communicate discharge plans to patients, families, and medical team.
• Delegate tasks and support transitional care staff.
• Collaborate with physicians, payers, and care team to meet regulatory requirements.
