Job summary

Qualifications

• Bachelor's Degree in Nursing or healthcare field is preferred.
• Associate's Degree in Nursing from an NLN/ACEN-accredited program is required.
• Current RN license in AZ or a state with licensure compact is required.
• Certification in Case Management (CCM) preferred.
• 3 years clinical hospital experience required.
• 5+ years as RN in Case Management preferred.
• 7 years as RN preferred.

Responsibilities

• Guides care coordination for high-risk acute care patients and facilitates discharge planning.
• Analyzes medical records to determine admission, treatment, and length of stay adherence.
• Advocates for patients/families and liaises with the care team to ensure timely services.
• Collaborates with physicians and coordinates resources to ensure timely care and safe discharge.
• Maintains department logs and databases and contributes to utilization review documents.
• Coordinates transitions of care with stakeholders to ensure smooth patient throughput.

Education

Associate's Degree in Nursing (NLN/ACEN)

Bachelor's Degree in Nursing or healthcare-related field
