Responsibilities

• The Case Manager coordinates health care decisions using a systematic approach to assure treatment plans that improve quality and outcomes.
• Coordinate care across the continuum and promote cost-effective care within the allotted time frame.
• Assess psychosocial needs and provide intervention as part of discharge planning process.
