Qualifications

• Active license in appropriate discipline (e.g., RN, SW, OT, PT, ST, Rehab Counseling).
• CCM® or ACM™ certification required OR must be obtained within two years of placement.

Responsibilities

• Coordinate patient care from admission to post-discharge with an interdisciplinary team.
• Lead or participate in plan-of-care decisions based on patient needs and preferences.
• Complete case management documentation and addendums accurately and timely.
