Qualifications

• RN: knowledge of case management or utilization review with a bachelor’s degree.
• RN must hold a current RN license in the state worked.
• Social Worker: MSW with LMSW/LCSW or MSW with license due within 6 months.

Responsibilities

• Coordinate discharge planning across the care continuum.
• Lead interdisciplinary discharge planning with the care team.
• Educate patients/families on post-acute care needs and resources.
• Maintain knowledge of payer benefits and community resources.
• Ensure timely communication with physicians, payers and care team.
• Possibly supervise other staff and support process improvements.
• Operate within established procedures with supervisory guidance as needed.
• Support Banner Health's patient-first care philosophy.
