Terros Health in Phoenix, AZ is seeking a dedicated Case Manager to join our interdisciplinary team serving individuals with Serious Mental Illness or general mental health needs at the Oak Recovery Health Center. This full-time role offers a 40-hour work week, day shift, with a comprehensive benefits package including 401K, PTO and health coverage.

Training is provided; new grads welcome and bilingual abilities are a plus.

Qualifications

• High school diploma or GED with 1.5 years of behavioral health experience.
• Typing proficiency minimum 25 wpm.
• Knowledge of Electronic Health Records; NextGen is a plus.
• Bilingual Spanish/ASL is desired.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services.
• Complete assessments, crisis plans and individualized service plans.
• Coordinate care with clinical team, family, and providers to implement plans.
• Conduct outreach and follow-up for crisis, missed appointments, and discharges.
• Manage caseload through assessment, planning, and evaluations.
• Pursue best practice outcomes for mental illness, including education, employment, and housing.

Seeking a Case Manager to join our interdisciplinary team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Oak Recovery Health Center.

Full-Time Employment: 40 hours per week. Monday - Friday: 8:00am - 5:00pm. Location: 4451 E. Oak St., Phoenix AZ, 85008. Full Benefits Package - Including 401K. Generous PTO/Sick Time (3+ weeks in Year 1).

Additional Language Differential Pay Available. Now Offering an Additional $1.50/hr SMI Differential.

No experience required with a Bachelors Degree in behavioral health - we will train you. New grads welcome!

Duties Include

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
• Completes assessments, crisis plans and individualized service plans.
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health providers, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented.
• Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
• Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluations.
• Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure.

Qualifications

• A high school diploma/GED with a minimum of 1.5 years of behavioral health employment experience, or
  • An associates degree in the field of medical or behavioral, or
  • A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
  • A bachelor's degree in the field of behavioral, or
  • A master's degree or higher in the field of behavioral health.
• Computer Skills: Proficient in Microsoft Word, Excel, PowerPoint, & Outlook. Must be able to pass typing assessment with minimum 25wpm.
• Knowledge in use of Electronic Health Records - NextGen is a plus.
• Bilingual in Spanish/ASL is desired.
• Excellent oral and written communication skills.
• Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.
• Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
• Must pass a TB Test.
