Job summary

Terros Health is seeking an on-call Case Manager in Phoenix, AZ to join an interdisciplinary clinical team. You will support individuals with SMI or general mental health diagnoses under Court Ordered Treatment at Priest Drive Recovery Health Center.

The role offers on-call employment, training, and opportunities to grow within a mission-driven health care organization. Bilingual Spanish is a plus, with additional language differential pay available.

Qualifications

  • GED/HS diploma with 1.5 years behavioral health experience or an associate degree in medical/behavioral.
  • An associates degree in the field of medical or behavioral health.
  • Bachelor's degree (not in behavioral health) with 1 year behavioral health experience, or a bachelor's in behavioral field.
  • Bachelor's degree in behavioral health.
  • Master's degree or higher in behavioral health.

Responsibilities

  • Assist clients with identifying treatment and recovery goals and provide supportive services.
  • Complete assessments, crisis plans and individualized service plans.
  • Collaborate with the clinical team to coordinate care with member, family, providers, and community resources.
  • Conduct outreach and follow-up for crisis interactions, missed appointments, and hospital discharges.
  • Perform all case management functions including assessments and service planning.
  • Pursue best practice outcomes for clients, including housing and community tenure.

Role Overview

Seeking a Case Manager to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Priest Drive Recovery Health Center.

Duties
  • Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals
  • Completes assessments, crisis plans and individualized service plans
  • Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented
  • Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
  • Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluations
  • Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure

Qualifications / Education & Experience
  • GED/High school diploma with a minimum of 1.5 years of behavioral health experience, or,
  • An associates degree in the field of medical or behavioral, or
  • A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
  • A bachelor's degree in the field of behavioral, or
  • A master's degree or higher in the field of behavioral health
  • Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred
  • Minimum typing speed of 25 wpm
  • Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy
  • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment
  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members
  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
  • Basic understanding of the DSM-V preferred
  • Certified Case Manager strongly preferred
  • Must pass background check, TB test and other pre-employment screening
