Job summary

Terros Health is hiring a Case Manager to join our interdisciplinary clinical team at the Priest Dr. Recovery Health Center in Tempe, AZ. The role supports individuals diagnosed with Serious Mental Illness or General Mental Health conditions under Court Ordered Treatment.

Day shift, 40 hours weekly, M-F 8:30a-5:30p, with full benefits and PTO. We offer an additional $1.50/hr SMI differential and a comprehensive benefits package, including 401K, dental/vision, and paid holidays.

Qualifications

  • GED/High school diploma with a minimum of 1.5 years of behavioral health experience.
  • An associates degree in the field of medical or behavioral health.
  • A bachelor’s degree (not in behavioral health) and 1 year of behavioral health experience.
  • A bachelor’s degree in the field of behavioral health.
  • A master’s degree or higher in the field of behavioral health.
  • Computer Skills: Word, Excel, Outlook, EMR (NextGen preferred)

Responsibilities

  • Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
  • Completes assessments, crisis plans and individualized service plans.
  • Works with the clinical team to engage, educate, communicate and coordinate care with members, families, providers and community resources to ensure services are implemented.
  • Conducts outreach and follow-up of services including crisis interactions, missed appointments, and hospital discharges.
  • Performs all case management functions including assessments and service planning, and pursuing further evaluations as needed.
  • Pursues best practice outcomes for individuals with mental illness including education, employment, housing, and community tenure.

Duties Include


  • Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals

  • Completes assessments, crisis plans and individualized service plans

  • Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualized service plan are implemented

  • Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.

  • Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for further or specialty evaluations

  • Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure

Qualifications

  • Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred

  • Valid AZ Driver License. Must have valid Arizona driver's license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health's driving policy

  • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment

  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members

  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions

  • Basic understanding of the DSM-V preferred

  • Certified Case Manager strongly preferred

  • Must pass background check, TB test and other pre-employment screening.

  • Must pass a typing assessment with a speed of at least 25 wpm.
