Job summary

Terros Health is seeking a Case Manager to join an interdisciplinary team serving individuals diagnosed with a Serious Mental Illness or General Mental Health diagnosis under Court Ordered Treatment at our South Mountain Recovery Health Center.

Full-Time Employment: 40 hours per week, Monday - Friday, 8:00am - 5:00pm, Location: 3540 E. Baseline Rd., Phoenix AZ, 85042. Full benefits including 401K, generous PTO/sick time, and additional pay differentials.

Qualifications

• High school diploma or GED with BH experience.
• Relevant BH education credentials as listed.
• Proficiency in MS Office and EHR systems.

Responsibilities

• Identify treatment and recovery goals and provide supportive services.
• Complete assessments, crisis plans and service plans.
• Coordinate with clinical team and providers to implement plans.
• Conduct outreach and follow-up to ensure resources are available.

Duties Include:

• Assists clients with identifying treatment and recovery goals and provides supportive services to help clients reach their goals.
• Completes assessments, crisis plans and individualizedservice plans.
• Works collaboratively with the clinical team to engage, educate, communicate and coordinate care with member, their family, behavioral health providers, medical and dental providers, community resources and others in ensuring that all services prescribed in the individualizedservice plan are implemented.
• Conducts outreach and follow-up of services including, but not limited to, crisis interaction, missed appointments, and hospital discharges to ensure adequate resources are available and in place.
• Performs all case management functions associated with caseload including completing the assessment and service planning processes; including identifying the need for furtheror specialty evaluations.
• Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure.

Qualifications:

• A high school diploma/GED with a minimum of 1.5 years of behavioral health employment experience
• An associates degree in the field of medical or behavioral
• A bachelor's degree (not in the field of behavioral health) and 1 year of behavioral health experience
• A bachelor's degree in the field of behavioral
• A master's degree or higher in the field of behavioral health
• Computer Skills: Proficient in Microsoft Word, Excel, PowerPoint, & Outlook. Must be able to pass typing assessment with minimum 25wpm.
• Knowledge in use of Electronic Health Records - NextGen is a plus.
• Bilingual in Spanish/ASL is desired.
• Excellent oral and written communication skills.
• Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.
• Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
• Must pass a TB Test.
