Job summary

Terros Health in Phoenix, AZ is seeking a Rehabilitation Specialist to join our interdisciplinary team serving individuals with Serious Mental Illness or general mental health diagnoses under Court Ordered Treatment at our Oak Recovery Health Center. You will work full-time, Monday through Friday, 8:00am–5:00pm, with a comprehensive benefits package including a 401K and generous PTO.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services
• Coordinate rehabilitation-related services and implement Individual Service Plans
• Document client progress toward recovery and maintain case notes
• Engage clients using motivational interviewing and supportive listening
• Collaborate with Employment Specialists, RSA counselors, and community providers to support employment and daily activities

Seeking a Rehabilitation Specialist to join our interdisciplinary team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Oak Recovery Health Center.

Full-Time Employment: 40 hours per week

Monday - Friday: 8:00am - 5:00pm

Duties Include:

• Assistclients with identifying treatment and recovery goals, and providesupportive services to help clients reach their employment, education, and community involvement goals.
• Implementservices identified inIndividual Service Plans, and coordinate rehabilitation-related services for clients served on the clinical team.
• Communicateand documentclient progress toward their recovery.
• Embracethe recovery philosophy and promotes a safe recovery environment for clients.
• Usebrief clinical interventions, Motivational Interviewing, and supportive listening skills to engage and support clients receiving services.
• Engage and providedirect supportto clients’ natural support systems.
• Workin partnership with co-located Employment Specialists and RSA Vocational Rehabilitation Counselors, community program providers, and other parties involved to assist clients inmaintaining employment, education, and other meaningful daily activities.

Qualifications:

• A high school diploma/GED with a minimum of 3 years of behavioral health employment experience, or
  • An associate's degree in behavioral health with at least 3 years of behavioral health experience, or
  • A bachelor’s degree (not in the field of behavioral health) and 1 year of behavioral health experience, or
  • A bachelor’s degree in the field of behavioral health and any behavioral health experience, or
  • A master’s degree or higher in the field of behavioral health.
• 1 year of supervised experience as a Rehabilitation or Employment Specialist, or Vocational Rehabilitation Counselor preferred.
• Computer Skills: Proficient in Microsoft Word, Excel, PowerPoint, & Outlook. Must be able to pass typing assessmentwith minimum 25wpm.
• Knowledge in use of Electronic Health Records - NextGen is a plus.
• Bilingual in Spanish/ASL is desired.
• Excellent oral and written communication skills.
• Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.
• Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
• Must pass a TB Test.
