Terros Health seeks a Case Manager to join the Oak Recovery Health Center in Phoenix. You will work with a multidisciplinary team to engage clients, assess needs, and develop individualized service plans. The role emphasizes coordination and supportive services for those with SMI, housing challenges, or court-ordered treatment.

Responsibilities include outreach, follow-up, and collaboration with medical, dental, and behavioral health providers to ensure comprehensive, person-centered care.

Competenze

• Bachelor's degree in behavioral health or related field required.
• 1.5 years behavioral health employment experience preferred.
• Valid Arizona driver’s license and driving experience required.
• Bilingual in Spanish/ASL desired.
