Terros Health is seeking a Rehabilitation Specialist to join our interdisciplinary team at the Oak Recovery Health Center in Phoenix, AZ. The role focuses on assisting clients with identifying treatment goals and supporting their employment, education, and community involvement goals.

You will implement services from Individual Service Plans, coordinate rehabilitation-related services, and document progress toward recovery.

Qualifications

• High school diploma or GED with behavioral health employment experience or the listed degree paths.
• Excellent written and verbal communication skills.
• Ability to pass typing assessment and use Windows office products.
• Valid Arizona driver’s license and clean driving record.

Responsibilities

• Assist clients with identifying treatment and recovery goals and support employment, education, and community involvement goals.
• Implement services identified in Individual Service Plans and coordinate rehabilitation services.
• Communicate and document client progress toward recovery.
• Promote recovery philosophy and a safe recovery environment.
• Use Motivational Interviewing and supportive listening to engage clients.
• Collaborate with Employment Specialists, RSA Counselors, and community program providers.

Must be able to pass typing assessment with minimum 25wpm.* Knowledge in use of Electronic Health Records - NextGen is a plus.* Bilingual in Spanish/ASL is desired.* Excellent oral and written communication skills.* Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.* Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.* Must pass a TB Test.
