Job summary

Terros Health is seeking a Rehabilitation Specialist to join an interdisciplinary team serving individuals with Serious Mental Illness or General Mental Health diagnoses under Court Ordered Treatment at Oak Recovery Health Center in Phoenix, AZ. The role is full-time, 40 hours per week, Monday–Friday, 8:00am–5:00pm, at 3540 E Baseline Rd, Unit #150.

You will assist clients in identifying treatment goals, coordinate rehabilitation services, and document progress within the clinical team, using

Qualifications

• A high school diploma or GED with a minimum of 3 years of behavioral health employment experience.
• Associate's degree in behavioral health with at least 3 years of behavioral health experience or a bachelor’s degree with 1 year of behavioral health experience.

Responsibilities

• Assist clients with identifying treatment and recovery goals and provide supportive services to help clients reach employment, education, and community involvement goals.
• Implement services identified in Individual Service Plans and coordinate rehabilitation-related services for clients on the clinical team.
• Communicate and document client progress toward recovery.
• Engage clients with Motivational Interviewing and supportive listening skills to promote recovery.

Must be able to pass typing assessment with minimum 25wpm.Knowledge in use of Electronic Health Records - NextGen is a plus.Bilingual in Spanish/ASL is desired.Excellent oral and written communication skills.Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.Must pass a TB Test.Physical demands of this position are representative of those that must be met by an employee to successfully perform the essential functions of this job.
