Job summary

PAM Health Specialty Hospital of Sarasota is seeking a Case Manager to lead interdisciplinary coordination of care for an inpatient population. You will review admission records to ensure appropriate resources and level of care, assess discharge needs, and coordinate the discharge plan with the care team.

Collaborating with nurses, therapists, social workers, and payors, you will support achievement of quality and financial outcomes while maintaining payor relationships and ensuring compliance

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Current BLS certification required.
• 3–5 years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

• Coordinate care for designated patient population across inpatient records.
• Assess discharge needs and coordinate discharge plan.
• Collaborate with interdisciplinary team to improve outcomes and payor relationships.
