PAM Health is seeking a Case Manager to coordinate interdisciplinary care for assigned patient populations. You will review inpatient admission records to ensure proper utilization of resources and determine appropriate levels of care, while coordinating discharge planning with the team.

Qualifications

• Must have licensure in the state where the hospital resides (preferred).
• Disciplines may include OT/PT/RN/RRT/SLP/LSW.
• Current BLS certification required.

Responsibilities

• Coordinate interdisciplinary care for designated patient populations.
• Review inpatient admission records to ensure appropriate utilization of resources and level of care.
• Assess discharge needs and coordinate the discharge plan.
• Collaborate with the interdisciplinary team to meet financial and quality outcomes.
• Maintain and strengthen payor relationships.
