Job summary

PAM Health in the United States seeks a Case Manager to coordinate interdisciplinary care for a designated patient population, reviewing inpatient records to ensure appropriate admission and resource utilization.

You will assess discharge needs, coordinate discharge planning with the team, and work to improve financial and quality outcomes while maintaining payor relationships. Current BLS certification and state licensure are required or preferred.

Qualifications

• Licensure in the state where hospital resides preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Current BLS certification required.

Responsibilities

• Coordinate the interdisciplinary care of a designated patient population and review inpatient admission records to ensure appropriate utilization of hospital resources.
• Assess discharge needs and coordinate the discharge plan.
• Collaborate with the interdisciplinary team to achieve desired financial and quality outcomes and maintain payor relationships.
