• Coordinate interdisciplinary care for a designated patient population.
• Review inpatient admission records to ensure proper utilization of resources and admission level of care.
• Assess discharge needs and coordinate appropriate discharge plan.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.

Education
