Job summary

PAM Health Rehabilitation Hospital of Jupiter seeks a Case Manager to coordinate care for an inpatient population. You will review admission records to ensure proper utilization of hospital resources and determine admission for the appropriate level of care.

You will assess discharge needs and coordinate the discharge plan, working with the interdisciplinary team to achieve financial and quality outcomes. You will also maintain and enhance payor relationships.

Qualifications

• Licensure in the state where PAM Health resides is preferred.
• Current BLS certification required.
• 3–5 years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

• Coordinate interdisciplinary care for a designated patient population.
• Review inpatient admission records to ensure proper utilization of hospital resources and admission decisions.
• Assess discharge needs and coordinate the discharge plan.
• Maintain and enhance payor relationships.
