Job summary

Telecare Corporation is seeking a Case Manager II in Norwalk, CA. Under direct supervision, you will act as the primary support and case manager for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their chosen community.

Shifts available: Full-Time, AM/NOC/PM, 8:00 AM - 4:30 PM, Monday - Friday with weekend and on-call coverage. Starting wage is $23.00/hour, with geographic differentials applied.

Qualifications

• High school diploma or GED; plus 2 years direct services in mental health or community services; or an Associates/Bachelor’s in Social Services.
• Valid driver’s license and personal vehicle insurance.
• Willingness to use personal vehicle to drive clients to appointments and meetings.

Responsibilities

• Under direct supervision, the Case Manager II provides primary support and case management for individuals with mental illness and co-occurring disorders to live as independently as possible in their community.
• Coordinate services, monitor progress, and advocate for client needs.
• Engage with clients in community settings and facilitate access to resources.
