Telecare Corporation is seeking a Case Manager II to work full-time in Oakland. You will serve as the primary support and case manager for individuals with mental illness and co-occurring disorders, helping them live as independently as possible in their community.

The role offers daytime shifts (9:00 AM - 5:30 PM, Monday to Friday), with wages starting around $23.00–$23.71 per hour, adjusted by geographic location and experience.

Qualifications

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services.

Responsabilités

• Serve as primary support and case manager for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their chosen community.
