Job summary

Telecare Corporation is seeking a Case Manager II to provide primary support and case management for individuals with mental illness and co-occurring disorders. Under direct supervision, you will help clients live as independently as possible in their chosen community.

The role features full-time daytime hours (8:30 AM–5:00 PM), with a starting wage range of $23.00–$27.63 per hour, adjusted by geographic location and experience. Telecare applies geographic differentials to its pay ranges.

Qualifications

• High School/GED or higher with relevant health service experience.
• Valid driver’s license and personal vehicle insurance in your name.
• Willingness to use personal/company vehicle to transport clients.

Responsibilities

• Provide primary support and case management for individuals with mental illness and co-occurring disorders.
• Assist clients to live as independently as possible in their community.
